The medical record is central to malpractice litigation and patient safety, making clear, accurate, timely documentation essential. Nearly 20% of malpractice cases involve documentation failures, which more than double indemnity payment risk. As EHRs improve access and communication, they also introduce risks—entry errors, incomplete records, and inconsistencies—that can undermine credibility in litigation. The result is a new reality: every note, timestamp, and digital artifact is part of a permanent, analyzable record that may be scrutinized years later by opposing experts and AI-assisted legal teams.

This presentation gives Oklahoma physicians a practical, risk-focused framework for keeping the medical record a strong defense—not a vulnerability.

Target Audience

The educational design addresses the needs and issues of healthcare providers including physicians, nurses, physician assistants, healthcare administrators, and office practice managers.
